The SAS/STAT User's Guide has the oldest references I could find for 
all
of the usual hierarchical clustering methods--see the chapter on the
CLUSTER procedure, pp. 530-536. More recent references can be found at
ftp://ftp.sas.com/pub/neural/clus_bib.txt
